Introduction to Forms

Dale City Animal Hospital offers clients the option of completing necessary forms at home. If you've ever tried to fill out a form while balancing a clipboard on your lap at the same time you're trying to keep your pet from visiting everyone else in the waiting room, then you'll appreciate the convenience of our online forms!

If you would like to register as a new Dale City Animal Hospital client, you may do so here. Existing clients may submit a change of contact information, or add a new pet (Please remember to make appointments at 703-670-6181.)

There are two ways to complete forms at home. You may submit forms online or print our forms to complete and submit to us in person or by fax. (If you wish to fax your form, please call us for the fax number.) Please read the directions below carefully to determine the form you need to use. If you have questions about using these forms, please contact our Webmaster at webmaster AT dalecityanimalhosp DOT com. (Please use the usual e-mail format. The address is written out to prevent spam.)

Our printable forms require that you have the free Adobe Acrobat Reader installed on your computer. If you do not have it, you may download it.

New Client Registration

Please follow these steps to register as a new client with Dale City Animal Hospital:

  1. Complete our New Client Registration Form
    Online Registration: You may use this online form if your appointment is scheduled for more than 48 hours from now.
    OR
    Print the form: If your appointment is less than 48 hours from now, print this form, complete it, and bring it with you to your appointment. You may also use this form if you simply prefer to register on paper rather than online.
  2. Print the Animal Medical Care Disclosure form. This form must be printed; it can not be submitted online. Please print, sign, and date the form and bring it with you when you come to your first appointment.

Forms for Existing Clients

Change of Information

If you're an existing Dale City Animal Hospital Client and you would like to change your contact information or add a new pet, you may use this form. You may choose to use our online form or our print form. If you're registering a new pet, please be sure to use the print form if you have an appointment for that pet in less than 48 hours. Otherwise, you may choose the method you prefer. The print form may be submitted in person or mailed or faxed to us. The mailing address is on the form. You may call us for our fax number.
